If you are located in a WSU Building on the School of Medicine Campus and need a network jack installed:

Fill out a Telephone Service Request from the WSU Telecommunications web site at http://computing.wayne.edu/phone/index.php.

The form can be dropped off or mailed to Facilities and Support Services - the office is located in Scott Hall Room 1102.  You can also fax the form to Facilities at 313-577-3434.
